HIPAA notice of privacy practices
This notice describes how medical information about you may be used and disclosed, and how you can get access to that information. Please review it carefully. A printed copy is available at the pharmacy counter on request.

Our responsibilities
We are required by law to protect the privacy of your protected health information, give you this notice of our legal duties and privacy practices, and follow the terms of the notice currently in effect.
How we may use and disclose your information
Treatment: we share information with your prescribers and other providers so your care is coordinated and safe.
Payment: we use your information to bill your insurance plan and collect payment for prescriptions and services.
Health care operations: we use information for quality review, staff training, licensing, and running the pharmacy.
As required by law: we may disclose information for public health reporting, oversight activities, legal proceedings, and other uses that state or federal law requires or permits.
Your rights
You have the right to inspect and get a copy of your records, ask us to correct information you believe is wrong, request an accounting of certain disclosures, request confidential communications at an alternate address or phone number, and ask us to restrict certain uses and disclosures.
You also have the right to receive a paper copy of this notice, even if you agreed to receive it electronically.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with the pharmacy or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will never retaliate against you for filing a complaint.
